Graduation

There is a series of forms to complete regarding graduation and program completion.  The MLS staff will obtain these forms and provide instructions for each form while you are in the Final Project seminar.  It is important to be mindful of the Graduate School’s dates and deadlines for each form.

1.  Commencement Attendance Approval Form

Submit this form to the Graduate School if you want to participate in commencement ceremonies in the fall or spring.

Due Date:  There are two commencement ceremonies per year (December and May).  The due dates for submission of this form are up to a few months prior to the ceremony (approximately March 1 for the May commencement and October 1 for the December commencement).  Check the Graduate School Web site for the actual date as soon as you know the term in which you will be graduating. 

This form requires the signature of your advisor(s) and the Director of Graduate Studies.  Obtain the necessary signatures and mail or fax the completed form to the MLS office.  The due dates are firm, so allow enough time to obtain the required signatures.

2.  Graduate School Application for a Degree Form

Due Date:  First working day of the month in which you want to graduate.

This form does not require any signatures but your own!  It can be sent directly to 200 Fraser Hall, or you can send it to us, and we will make a copy for your file and get it to Fraser (again, allow time for this).

3.  Examination Report Master's Degree Form

 Due Date:  The last working day of the month in which you want to graduate.

This is the official document that indicates you have finished all your course work and your final project and, therefore, your master's degree.  It needs the signature of the DGS only.

The MLS staff will have this form completed for you.  We will obtain the DGS's signature and submit the form to the Graduate School on your behalf upon receipt of your signed and bound final project.  It may take a few days to obtain the signature and forward the form to the Graduate School, so allow enough time toward the end of the month to complete this process.
